Please read the following terms of service before proceeding to complete registration.

1. By proceeding to registration you certify that you are a dental professional or are an authorized representative of a dental professional.
2. In creating your account you must provide complete and accurate information. 
3. You understand that any information provided in the creation of your profile account could be made public, with the exception of your email address. Furthermore, you grant Dental Portal license to use your information to promote both your practice, as well as dentalportal.com. Please read our Privacy Policy for additional information regarding the exchange information.
4. You are responsible for your account and should therefore take precaution to safeguard your password, which can be changed anytime, as necessary. You should notify dentalportal.com immediately if you feel there has been a security breach beyond your control.
5. In creating your account, you may provide up to one logo and one photo. If these are provided, you grant Dental Portal license to use these images in association with your profile and/or to promote your profile or for promotion of the dentalportal.com.

General Use of the Website as is applicable to Dental Professional Profiles

1. Dental professional profiles and the information provided therein is for the purpose of providing information about your practice and services to the general public.
2. Accounts may not be transferred, resold or used for commercial profit.
3. Posting information about another practice or dental professional is prohibited.
4. Posting misleading information about your practice is prohibited. If information regarding your practice changes, such as your address or services provided, your profile should be updated within 30 days of those changes.
5. Posting logos or images for which you do not have license to is prohibited.
6. Posting inappropriate information or images is prohibited. Dental Portal has absolute discretion in defining "prohibited."
7. Attempting to circumvent security systems and measures in any way is prohibited.

8. Warranty Disclaimer

YOU AGREE THAT YOUR USE OF THE Dental Portal WEBSITE SHALL BE AT YOUR SOLE RISK. TO THE FULLEST EXTENT PERMITTED BY LAW, Dental Portal, ITS OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS DISCLAIM ALL WARRANTIES, EXPRESS OR IMPLIED, IN CONNECTION WITH THE WEBSITE AND YOUR USE THEREOF. The Website is controlled and offered by Dental Portal from its facilities in the United States of America. Dental Portal makes no representations that the Dental Portal Website is appropriate or available for use in other locations. Those who access or use the Dental Portal Website from other jurisdictions do so at their own volition and are responsible for compliance with local law.

10. Indemnity

You agree to defend, indemnify and hold harmless Dental Portal, its parent corporation, officers, directors, employees and agents, from and against any and all claims, damages, obligations, losses, liabilities, costs or debt, and expenses (including but not limited to attorney's fees) arising from: (i) your use of and access to the Dental Portal Website; (ii) your violation of any term of these Terms of Service; (iii) your violation of any third party right, including without limitation any copyright, property, or privacy right; or (iv) any claim that one of your User Submissions caused damage to a third party. This defense and indemnification obligation will survive these Terms of Service and your use of the Dental Portal Website.

11. Ability to Accept Terms of Service

You affirm that you are of appropriate age and of sound mind and that you are a legal representative of the practice for which you are registering.

12. Assignment

These Terms of Service, and any rights and licenses granted hereunder, may not be transferred or assigned by you, but may be assigned by Dental Portal without restriction.

13. General

You agree that: (i) the Dental Portal Website shall be deemed solely based in Arizona; and (ii) the Dental Portal Website shall be deemed a passive website that does not give rise to personal jurisdiction over Dental Portal, either specific or general, in jurisdictions other than Arizona. These Terms of Service shall be governed by the internal substantive laws of the State of Arizona, without respect to its conflict of laws principles. Any claim or dispute between you and Dental Portal that arises in whole or in part from the Dental Portal Website shall be decided exclusively by a court of competent jurisdiction located in San Mateo County, California. These Terms of Service, together with the Privacy Notice at http://www.dentalportal.com/privacy and any other legal notices published by Dental Portal on the Website, shall constitute the entire agreement between you and Dental Portal concerning the Dental Portal Website. If any provision of these Terms of Service is deemed invalid by a court of competent jurisdiction, the invalidity of such provision shall not affect the validity of the remaining provisions of these Terms of Service, which shall remain in full force and effect. No waiver of any term of this these Terms of Service shall be deemed a further or continuing waiver of such term or any other term, and Dental Portal's failure to assert any right or provision under these Terms of Service shall not constitute a waiver of such right or provision. Dental Portal reserves the right to amend these Terms of Service at any time and without notice, and it is your responsibility to review these Terms of Service for any changes. Your use of the Dental Portal Website following any amendment of these Terms of Service will signify your assent to and acceptance of its revised terms. YOU AND Dental Portal AGREE THAT ANY CAUSE OF ACTION ARISING OUT OF OR RELATED TO THE Dental Portal WEBSITE MUST COMMENCE WITHIN ONE (1) YEAR AFTER THE CAUSE OF ACTION ACCRUES. OTHERWISE, SUCH CAUSE OF ACTION IS PERMANENTLY BARRED.

14. Account Termination

Your account may be immediately terminated without explanation by Dental Portal at any time. Should you decide to terminate your account with Dental Portal provide a written notice and allow 30 days for removal from the system. Fees paid to Dental Portal are non-refundable.
